This Privacy Policy describes how Pharmaprodia Compounding Pharmacy ("Pharmaprodia," "we," "us," or "our") collects, uses, shares, and secures the personal information you provide to us, whether online, over the phone, in written correspondence, or in person at one of our locations. At Pharmaprodia Compounding Pharmacy, we take your privacy seriously. We are committed to maintaining your trust by protecting personal information we collect about you, our customer. This Privacy Policy describes how we treat personal information received about you from our websites such as pharmaprodia.com (the “Website”), through the software applications made available by Pharmaprodia for use on or through computers and mobile devices (the “Apps”), as well as through the provision of Pharmaprodia’s pharmacy services (collectively, the “Services”).

When you visit our pharmacy location to pick up a prescription, we will verify your identity by requesting a government-issued ID.

This information is required for us to fill your prescription and provide pharmacy services to you. We only access the minimum amount of medical information necessary from other healthcare providers.

- Insurance Information: If you use insurance benefits to pay for some or all of your prescription, we will collect your insurance carrier, member ID number, and group number. This information is used only to bill your insurance plan for covered prescriptions and pharmacy services. 

- Product and Service Usage Information: We collect information about how you interact with and use our Website, Apps, and Services. This includes pages visited, buttons clicked, features utilized, transaction details, and referral source or links used to navigate to our Website or Apps. 

- Device Information: We may collect information about the computer or mobile device you use to access our Website or Apps, including IP address, unique device identifier, browser type, operating system, mobile network carrier, and web browser cookies. This information helps customize your viewing experience based on your device.

This includes Contact/Identifying Information, Health Information, Insurance Information, and Preference Information.

- Process payments. We use Contact/Identifying Information and Insurance Information to bill your health plan or process payments for self-pay prescriptions and other services.

- Communicate about products, services and promotions. We may send you newsletters, emails, or other marketing messages using Contact/Identifying Information and Preference Information, based on your selected preferences. You can opt out anytime. 

- Analyze use of Website, Apps, and Services. We use Website Usage Information, Device Information, and Preference Information to help analyze trends and usage of our digital platforms. This allows us to improve the user experience and develop new offerings.

- Protect patient safety. We may use Health Information and Contact/Identifying Information to protect patient health and safety and to address product issues or concerns raised by patients, healthcare providers, or regulatory bodies. 

- Comply with legal and regulatory requirements. We use Contact/Identifying Information, Health Information, and Insurance Information to comply with laws that regulate our pharmacy services. This includes verifying identity, detecting and preventing fraud, and responding to regulatory oversight for pharmacy services.

- Health Care Providers: We share Health Information with the doctors, nurses, hospitals, clinics, nursing homes, and other health care providers involved in your health care. This assists your other health care providers in making better treatment decisions for you.

- Service Providers: We partner with third party service providers to perform services on our behalf. This includes payment processors, supply chain and logistics companies, marketing service providers, and technology vendors. These service providers are given limited access to your information needed to perform their function, but are contractually required to protect and secure your information consistent with this Privacy Policy. 

- Group Purchasing Organizations: We provide pricing information and transaction data to group purchasing organizations that we contract with to negotiate and administer pricing and product availability. We only share information necessary for them to validate the pricing and administration of their contracts.

- PBMs and Health Plans: We share Health Information, Contact/Identifying Information, and Insurance Information with Pharmacy Benefit Managers and health insurance providers for coordination of benefits and payment. 

- Government Agencies and Law Enforcement: We may share personal information with government agencies or law enforcement to the extent we reasonably believe is required by law. This includes responding to court orders, warrants, or subpoenas to prevent harm or financial loss; report criminal activity or threats to public safety; or investigate and prosecute fraud.

- Sale or Acquisition: In the event we go through a business transition such as a merger, acquisition, bankruptcy or other transaction, we may need to transfer your information to a successor entity.  

Your Choices and Rights

- Objecting to or Opting Out of Communications: You may opt out of receiving marketing emails, text messages, telephone calls, and postal mail from us at any time by using the unsubscribe link within the message, changing your communication preferences in your profile, or by contacting us directly. 

- Requesting Deletion of Your Information: You may request that we delete your Contact/Identifying Information, Health Information, and other personal data. We will comply unless certain information is required to be retained for legal or compliance purposes.  

- Obtaining Your Information: In some jurisdictions, you have a right to request a copy of the personal information we maintain about you. We will provide you with a copy of your personal information in electronic form to you after validating your identity.

- Revoking Consent: If you revoke your consent for us to use or share your Health Information, Insurance Information or other sensitive personal information, we may no longer be able to provide pharmacy services to you. 

How We Protect Your Personal Information

Pharmaprodia takes data security seriously, and we use physical, technical, and administrative safeguards to protect your data. All personal information is encrypted in transit and at rest, and our systems are monitored and routinely tested for proper security controls. Access to personal data is restricted to employees who need it to perform services, and all employees are bound to strict contractual confidentiality obligations.

While we follow industry standards to protect your information, no security program can prevent all potential loss or misuse of data. Please notify us immediately if you become aware of any unauthorized access of your personal information.

Information about Children

The pharmacy services, Website, Apps, and digital offerings provided by Pharmaprodia are not directed to individuals under the age of 18, and we do not intentionally gather personal data from children under 18. If we are made aware that a child under 18 has submitted personal information without parental consent, we will take steps to delete that information.

External Websites and Links

The Website or Apps may contain links to third party websites that are outside our control and not covered by this Privacy Policy. Pharmaprodia is not responsible for the privacy practices or policies of third party websites that may be linked from our digital platforms.  

California Resident Rights
